Job summary

Qualifications

• Community Health Worker certification is required.
• High school diploma or GED; or equivalent experience.
• Fully bilingual in Spanish and English.
• Experience or training helping people and connecting them to resources.

Responsibilities

• Coordinate care for patients with complex needs and ensure access to services.
• Develop and monitor individualized care plans.
