Job summary

San Bernardino County is seeking an Enhanced Care Management (ECM) Care Coordinator to support Arrowhead Regional Medical Center, the Department of Behavioral Health, and the Department of Public Health. You will work across county clinics, community health settings, and field environments to connect patients with needed resources, including after-hours or nontraditional schedules.

Qualifications

Multilingual abilities including ASL and multiple languages are beneficial.
Medical Assistant program is required.
Six months of full-time work experience assisting medical professionals is preferred.

Responsibilities

Conduct a daily scrub of ECM patients to identify visits and engage during the visit.
Carry a caseload of ECM patients, providing care coordination and referrals for physical, mental, and social support services.
Participate in SCR meetings and present patients for review and follow-up.
Build relationships with patients and develop improvement plans for health needs.
Conduct Comprehensive Health Assessments and PHQ-9 as required by ECM.
Ensure engagement and completion of tasks related to ECM metrics.
