• Bachelor's degree in one of the fields listed under Qualifications.
• Minimum 1 year of experience working with infants, toddlers, or children with disabilities.

Responsibilities

• Serve as primary contact for families and connect them with needed services.
• Coordinate evaluations and IFSP/FSP development and reviews.
• Assist families in navigating Medicaid, IDEA Part C, and other programs.
• Monitor service delivery and coordinate across providers and agencies.
• Complete service notes and ensure compliance with documentation requirements.
• Collaborate with families, providers, and multidisciplinary teams.
