Job summary

Care Lync is seeking a Supports Coordinator to coordinate services for children from birth to age 6 with developmental delays or disabilities and to support families in understanding their rights and accessing coordinated, high-quality early intervention supports.

You will serve as the primary contact for families, facilitate evaluations and IFSP development, and work with a multidisciplinary team to promote child development and family capacity, including monitoring and documentation.

Qualifications

  • Bachelor's degree in Social Work, Psychology, Sociology, Early Childhood Education or Special Education, Child Development, Family Studies or Health-related field.
  • Minimum 1 year of experience working with infants, toddlers, or children with disabilities.
  • Preferred CPR/First Aid certification; if not held, support to obtain on hire.

Responsibilities

  • Serve as the primary contact for families and assist in accessing services.
  • Coordinate evaluations, IFSP/FSP development and reviews, and multidisciplinary team meetings.
  • Support families in understanding their rights and accessing services in natural environments.
  • Monitor service delivery, complete service notes, and ensure Medicaid and IDEA Part C compliance.
  • Collaborate with families, providers, and agencies; participate in team planning and communication.
