Job summary

• Bachelor's degree in one of the listed fields and a minimum of 1 year of experience with infants, toddlers, or children with disabilities.
• Experience with Early Intervention, IDEA Part C, or pediatric services is preferred.
• Knowledge of IFSP/FSP development and child developmental milestones.

Responsibilities

• Serve as primary point of contact for families and coordinate service delivery.
• Assist families in accessing needed services and scheduling appointments.
• Coordinate evaluations and develop IFSP/FSP with multidisciplinary teams.
• Educate families on rights, safeguards, and caregiver empowerment.
• Monitor services for alignment with IFSP/FSP and coordinate across systems.
• Complete documentation and ensure Medicaid/IDEA Part C compliance.
