Job summary

Atrinity Home Health, part of the LHC Group and within the Optum family, is seeking a PRN Physical Therapist for home health in Norwalk, CT. You will assess functional status, determine a care plan, and implement interventions to improve mobility, endurance, and comfort.

You will evaluate patients, coordinate with the supervising nurse and physician, and document progress. Must hold current CT PT licensure and a valid driver's license; CPR certification is preferred or obtainable within 90 days

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• CPR certification preferred or able to complete within 90 days of hire.

Responsibilities

• Evaluates the patient's functional status and physical therapy needs; assists in plan of care development.
• Provides services within scope of practice and plan of care; coordinates with health care team.
• Observes and reports patient response to treatment and changes in condition.
• Treats patients to relieve pain, restore function, and maintain maximum functioning.
• Educates patient and family about plan of care, limitations, and progress.
• Visits patients according to Plan of Treatment and submits progress notes.
• Completes all evaluations and develops PT plan of care per state guidelines.
• Reports outcomes, goals, and frequency of care.
