Job summary

Optum's Home Health of Wilson, part of LHC Group, is seeking a PRN Physical Therapist to assess and plan care for patients in their homes. You will evaluate functional status, develop treatment plans, and deliver therapy to improve mobility and pain, coordinating with the care team.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.

Responsibilities

• Evaluates the patient's functional status and physical therapy needs.
• Assists physician in development of the therapy plan of care.
• Provides services within the scope of practice per state laws and plan of care.
• Observes and reports patient response to treatment and changes in condition.
• Treats patients to relieve pain, restore function, and maintain maximum functioning.
• Instructs patient and family/caregivers regarding plan of care and progress.
• Completes evaluations and documents progress notes per policy.
• Reports outcomes and planned frequency of care.
