Job summary

Atrinity Home Health, part of LHC Group and affiliated with Optum, is seeking a Physical Therapist for home health services. You will perform assessments, develop a treatment plan, and deliver interventions to improve mobility and function in patients’ homes.

The role requires current PT licensure, CPR, a valid driver’s license and vehicle access, and the ability to work in diverse home settings. Comprehensive benefits are offered within UnitedHealth Group’s care network.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current CPR certification.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client.

Responsibilities

• Evaluates the patient's functional status and physical therapy needs, assists physician in plan of care.
• Provides services within scope of practice per state laws and plan of care, coordinating with health care team.
• Observes and reports patient responses to treatment and changes in condition to supervising nurse/physician.
• Treats patients to relieve pain, develop/restores function, and maintain maximum functioning.
