Job summary

Atrinity Home Health, part of the LHC Group and within the UnitedHealth Group family, seeks a PRN Physical Therapist for home health in Connecticut. You will assess patient needs, develop and implement plans of care, and collaborate with the health care team to improve mobility and function.

You will work to maintain high-quality patient care, document progress, and ensure plan of care alignment with state guidelines while enjoying a comprehensive benefits package and opportunities for growth

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Current CPR certification or ability to complete within 90 days of hire.

Responsibilities

• Evaluates the patient's functional status and physical therapy needs.
• Provides services within the scope of practice as defined by state laws.
• Observes and reports patient responses to treatment to the supervising nurse/physician.
• Treats patients to relieve pain, develop/restore function, and maintain maximum functioning.
• Instructs patients and family/caregivers on plan of care and progress toward goals.
• Completes evaluations and develops PT plan of care per state guidelines.
