Job summary

Elite Home Health, a part of LHC Group within the Optum family, is seeking a PRN Physical Therapist to evaluate patients in home health, develop treatment plans, implement interventions, and monitor progress to improve mobility and function. This role requires state PT licensure, a valid driver's license with vehicle insurance or access to dependable transportation.

CPR certification is preferred (or obtained within 90 days).

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.

Responsibilities

• Evaluates patient functional status and PT needs according to plan of care.
• Provides PT services within the scope defined by state laws and plan of care.
• Documents patient progress and communicates with supervising nurse/physician.
