Job summary

Atrinity Home Health, part of the Optum family, seeks a Physical Therapy Assistant to support in home care settings in Connecticut. You will work under a supervising PT, following state regulations, and provide patient-centered therapy to improve mobility and daily activities.

Responsibilities include conducting prescribed exercises, observing patient responses, and coordinating with nurses and physicians. Must be able to travel to patient homes and accommodate weekend shifts.

Qualifications

• Licensed or certified to assist in physical therapy in the state of practice.
• Valid driver's license and reliable transportation.
• Weekend rotation availability.

Responsibilities

• Follow state laws for physical therapy in home care and work under supervising PT.
• Instruct and aid patients in exercises, gait, ADLs, and training.
• Observe and report patient responses to treatment to PT, nurse, or physician.
• Coordinate care with other healthcare team members.
• Educate patient, family, and caregivers on therapy within PT assistant scope.
• Function effectively in diverse home settings and with all clients.
