Job summary

Atrinity Home Health, part of LHC Group and the Optum family, is hiring a PRN Physical Therapist for home health assessments and treatment planning. You will evaluate functional status, determine plans of care, and deliver interventions to improve patient well-being, with documentation and coordination with the health care team.

The role emphasizes flexible scheduling, patient-centered care, and collaboration with physicians and nurses to achieve optimal outcomes in a home health setting.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Preferred: CPR certification or ability to complete within 90 days of hire.

Responsibilities

• Evaluates the patient's functional status and physical therapy needs.
• Assists the physician in the development of the therapy plan of care.
• Provides services within the scope of practice as defined by state laws and plan of care.
• Observes and reports patient responses to treatment and changes in condition.
• Treats patients to relieve pain, develop and restore function, and maintain maximum functioning.
• Instructs patient and family/caregivers regarding plan of care and progress toward goals.
• Visits patients according to Plan of Treatment and submits progress notes per policy.
• Develops PT plan of care within state guidelines.
• Reports outcomes and frequency of care.
