Job summary

Optum, in collaboration with Patient Care (LHC Group), seeks a Physical Therapy Assistant to provide in-home care across Stamford, Greenwich, Norwalk and Wilton, CT. You will implement treatment plans, educate patients and families, and work with the supervising PT to improve overall well-being in a community setting.

Qualifications

• Licensed PT Assistant in CT, eligible to practice.
• Ability to perform physical tasks required for patient care.

Responsibilities

• Follow supervision plan set by PT and adhere to state laws.
• Instruct patients in active and passive exercises and functional activities.
• Observe and report patient responses; coordinate with healthcare team.
• Educate patient, family, and caregivers on therapy within PTA scope.
• Function effectively in diverse home-care settings.
