Qualifications

• Licensed Physical Therapist Assistant (PTA) - Illinois.
• BLS within 30 days - American Heart Association.
• Complete required CE for DNV Hip and Knee Replacement.
• Pass the board exam on the next available date.

Responsibilities

• Educates patients, family, caregivers, students and staff using effective methods.
• Follows the therapist’s plan of care and documents communications in daily notes.
• Completes documentation per CTSCP67 policy and participates in annual education for DNV requirements.
• Collaborates with care team and documents changes in patient status and discharge needs.
