Job summary

Carle Health is seeking a Licensed Physical Therapy Assistant to implement and adapt a patient care plan under a PT's supervision across age groups. The role emphasizes education, documentation, and collaboration with the care team.

The incumbent will follow established plans, adjust treatments with clinical judgment, and ensure accurate daily notes in the EMR. A strong readiness for continuing education and compliance with HIP and accreditation requirements is essential.

Qualifications

• Associate's Degree in Physical Therapy Assistant required.
• Licensed Physical Therapist Assistant (PTA) in good standing per IDFPR.
• Basic Life Support (BLS) certification within 30 days – AHA.
• Board Eligible or must complete board exam on next available date; attend required CE for acute stroke care.

Responsibilities

• Educates patients, family, caregivers, students and other healthcare providers using effective teaching methods.
• Follows therapist's plan of care; adjusts treatment based on patient response and available evidence after communicating with the evaluating therapist and documenting in daily notes.
• Produces documentation in accordance with the Clinical Documentation Policy (CTSCP67).
• If applicable, completes annual education to comply with DNV Hip and Knee Replacement Program requirements.
• Follows treatment plan of care established by PT and provides interventions including therapeutic exercise, gait and balance training, ultrasound; administers electrical stimulation; aquatic therapy; completes daily documentation and charging in therapy EMR; coordinates with PT on treatment interventions; communicates progress to referring provider.
