Job summary

Carle Health seeks a licensed Physical Therapist to evaluate and treat patients of all ages, designing plans of care and adjusting them as needed. You will educate patients and families, document outcomes, and collaborate with providers.

Qualifications

• Bachelor's degree in Physical Therapy and licensure as a PT.
• Current Basic Life Support (BLS) certification.
• Ability to complete required continuing education for acute stroke care.

Responsibilities

• Evaluate and treat patients across ages, adjusting plans of care as needed.
• Educate patients and families using effective teaching methods.
• Develop and adjust care plans based on patient response and evidence.
• Document care in the EMR in accordance with policies.
• Perform patient transfers with safe body mechanics.
• Coordinate with referring providers on progress and treatment recommendations.
• Supervise and provide feedback for support personnel involved in care.
• Ensure compliance with program requirements and annual training.
• Evaluate impairments and establish goals for orthopedic/neurologic conditions.
