JAG Physical Therapy is seeking a dedicated Physical Therapist in New York. The role involves evaluating patients, creating individualized treatment plans, and delivering care that maximizes functional independence.

Qualifications

• Bachelor’s or Master’s Degree required; Doctorate preferred.
• Licensed Physical Therapist in NY/NJ.
• Completed state CEU requirement in appropriate state (NY, NJ).
• Experience with EMR software preferred.

Responsibilities

• Collects and records patient care data per guidelines and as per third-party payers.
• Record daily billing and follow productivity guidelines.
• Prepare for evaluations per department policies and standards.
• Establish treatment plans based on evaluations and goals; collaborate with the team.
• Identify need for consultations and refer to other health professionals as needed.
• Oversee PTA, ATC, and PT aides; provide coaching and guidance.
