Job summary

JAG Physical Therapy is seeking a dedicated Physical Therapist in New Jersey to assess patients, design effective treatment plans, and guide recovery while maintaining confidentiality and a positive, energetic environment.

You will utilize your training to improve pain, function, and independence, perform evaluations, document care, and collaborate with an interdisciplinary team to optimize outcomes.

Qualifications

• Bachelor’s or Master’s Degree in Physical Therapy required or Doctorate preferred.
• Licensed Physical Therapist in NJ.
• Completed CEU requirement in NY/NJ.
• Experience with EMR software preferred.
• Excellent oral and written communication.
• Strong clinical decision-making skills.
• Positive work ethic and dedication to patient success.
• Basic computer skills; team player, independent as needed.

Responsibilities

• Collect and record patient care data per guidelines and regulations.
• Record daily billing and maintain productivity.
• Prepare for evaluations per department policies and standards.
• Select and administer tests and measures.
• Formulate diagnosis, goals, prognosis and plan of care.
• Establish treatment plans and collaborate with the interdisciplinary team.
• Refer to other professionals as needed and complete discharge plans.
• Educate patients and families; provide training.
• Oversee PTA, ATC, and PT aides.
