Job summary

JAG Physical Therapy is seeking a forward-thinking Physical Therapist in New Jersey to assess patients and craft effective, individualized treatment plans. You will document evaluations, progress, and plans of care, while maintaining confidentiality and fostering a high-energy, supportive environment to help patients regain function and reduce pain.

The role requires a state license and a degree in Physical Therapy; EMR experience is preferred.

Qualifications

• Bachelor’s or Master’s degree required; Doctorate preferred.
• Licensed Physical Therapist in the state.
• Completed CEU requirement in NY or NJ state.
• Experience with EMR software preferred.
• Excellent oral and written communication.

Responsibilities

• Collect and record patient care data per guidelines and payers.
• Conduct initial evaluation, daily notes, progress reports, and discharge plans.
• Record daily billing and follow productivity guidelines.
• Prepare for evaluations per department policies and standards.
• Select and administer tests and measures; formulate diagnosis and plan of care.
• Establish and adjust treatment plans; collaborate with the team.
• Referrals when needed and discharge planning; patient education.
• Train patients and families in ADLs/IADLs and rehab activities.
• Oversee PTA/ATC/PT aides; participate in billing and coding seminars.
