Job summary

Carle Health is seeking a driven Physical Therapist to evaluate and treat patients of all ages, adjusting plans of care based on patient response and evidence. The role includes educating patients and families, documenting outcomes, and coordinating with the care team to ensure safe transfers and optimal recovery.

Licensed PT (IDFPR) and BLS within 30 days are required, with continued education in stroke care as applicable.

Qualifications

• Licensed Physical Therapist (PT) required.
• Continued education related to acute stroke care may be required.
• BLS certification within 30 days from hire.

Responsibilities

• Evaluate and treat patients of all ages; develop and adjust plans of care.
• Educate patients and families; document progress and discharge plans.
• Perform examinations, determine PT diagnoses, and select appropriate tests.
• Coordinate with staff and supervisors; use therapy EMR for documentation.
• Ensure safe patient transfers and apply modalities as appropriate.
• Communicate progress to referring providers and adjust treatment as needed.
