Job summary

Talented Medicals Solution is seeking a compassionate Physical Therapist (PT) to provide high-quality rehabilitation services. You will evaluate patients, develop individualized plans, implement interventions, and monitor progress to restore mobility and reduce pain.

Collaborating with physicians and interdisciplinary teams, you will ensure safe, evidence-based care while maintaining thorough documentation and patient education for discharge readiness.

Qualifications

• Active Physical Therapist license in the state of practice.
• BLS/CPR certification preferred or required per facility policy.
• DPT degree or equivalent as required by state regulations.
• Experience in acute care, outpatient, inpatient rehab, skilled nursing, home health, or specialty settings preferred.
• Strong clinical assessment, communication, documentation, and patient education skills.
• Ability to work independently and with interdisciplinary teams.

Responsibilities

• Evaluate patients and develop individualized treatment plans based on functional needs and physician referrals.
• Provide interventions to improve mobility, strength, balance, flexibility, coordination, pain management, and functional independence.
• Monitor progress, modify treatment plans, and document evaluations, treatments, and outcomes.
• Educate patients, families, and caregivers on plans, home exercises, prevention, and discharge.
• Collaborate with physicians and healthcare teams to coordinate patient care and outcomes.
• Maintain accurate clinical documentation per policies and regulatory requirements.
• Ensure compliance with safety, infection control, and quality standards.
• Treat patients across care settings and age groups as required by the facility.
• Perform other assigned job duties.
