Job summary

Talented Medical Solutions is seeking a compassionate Physical Therapist (PT) to provide high-quality rehabilitation services in Indiana. The PT will evaluate patients, develop individualized plans, implement therapies, and monitor progress to restore mobility and function.

The role requires licensure in Indiana, a DPT, and strong collaboration with physicians and the care team. The position emphasizes patient education, safety, and evidence-based practice across acute care and other settings.

Qualifications

• Active PT license in Indiana.
• BLS/CPR certification preferred or required per facility policy.
• DPT degree or equivalent per state regulations.
• Experience in acute care, outpatient, inpatient rehab, SNF, home health or related settings preferred.
• Strong clinical assessment, communication, documentation, and patient education skills.

Responsibilities

• Evaluate patients and develop individualized treatment plans based on functional needs and referrals.
• Provide interventions to improve mobility, strength, balance, flexibility, coordination, pain management, and independence.
• Monitor progress, modify plans as needed, and document evaluations, treatments, and outcomes.
• Educate patients, families, and caregivers on treatment plans, home exercise programs, and discharge instructions.
• Collaborate with physicians and care teams to coordinate patient care and outcomes.
• Maintain accurate and timely clinical documentation per policies and regulations.
• Ensure compliance with patient safety, infection prevention, and quality standards.
• Treat patients across assigned care settings and age groups as required.
• Perform other duties as assigned.
