Job summary

Talented Medical Solutions is seeking a compassionate Physical Therapist (PT) to provide patient-centered rehabilitation in a healthcare setting in Globe, AZ. The PT will evaluate patients, develop individualized treatment plans, and implement therapeutic interventions to restore mobility and function.

You will collaborate with physicians and interdisciplinary teams, maintain thorough documentation, and educate patients and families on home exercise programs and discharge planning.

Qualifications

• Active PT license in the state of practice.
• DPT degree or equivalent educational requirements as required by state regulations.
• Previous experience in acute care, outpatient, inpatient rehabilitation, skilled nursing, home health, or specialty-specific settings preferred.
• Strong clinical assessment, communication, documentation, and patient education skills.
• Ability to work independently and collaboratively within an interdisciplinary healthcare team.

Responsibilities

• Evaluate patients and develop individualized treatment plans based on functional needs and physician referrals.
• Provide therapeutic interventions to improve mobility, strength, balance, flexibility, coordination, pain management, and functional independence.
• Monitor patient progress, modify treatment plans as appropriate, and document all evaluations, treatments, and outcomes accurately.
• Educate patients, families, and caregivers regarding treatment plans, home exercise programs, injury prevention, and discharge recommendations.
• Collaborate with physicians and interdisciplinary healthcare teams to coordinate patient care and achieve optimal rehabilitation outcomes.
• Maintain accurate and timely clinical documentation in accordance with facility policies and regulatory requirements.
• Ensure compliance with patient safety, infection prevention, and quality assurance standards.
• Treat patients across assigned care settings and age groups as required by the facility.
• Perform other job-related duties as assigned.
