Job summary

Talented Medical Solutions seeks a compassionate Physical Therapist (PT) to evaluate patients, develop individualized treatment plans, and provide evidence-based rehabilitation in a healthcare setting. You will monitor progress, adjust plans as needed, and document outcomes to support safe, effective care.

Qualifications

• Active PT license in the state of practice.
• BLS/CPR certification required or preferred per facility policy.
• DPT degree or equivalent as required by state regulations.
• Experience in acute care, outpatient, inpatient rehab, SNF, home health, or specialty settings.
• Strong clinical assessment, communication, documentation, and patient education skills.

Responsibilities

• Evaluate patients and develop individualized treatment plans based on functional needs and physician referrals.
• Provide therapeutic interventions to improve mobility, strength, balance, flexibility, coordination, pain management, and functional independence.
• Monitor patient progress, modify treatment plans as appropriate, and document all evaluations, treatments, and outcomes accurately.
• Educate patients, families, and caregivers regarding treatment plans, home exercise programs, injury prevention, and discharge recommendations.
• Collaborate with physicians and interdisciplinary healthcare teams to coordinate patient care and achieve optimal rehabilitation outcomes.
• Maintain accurate and timely clinical documentation in accordance with facility policies and regulatory requirements.
• Treat patients across assigned care settings and age groups as required by the facility.
• Perform other job-related duties as assigned.
