TEEMA Solutions Group is seeking a compassionate Physical Therapist (PT) to evaluate patients, develop individualized treatment plans, and implement therapeutic interventions to restore mobility and reduce pain in a healthcare setting.

You will collaborate with physicians and interdisciplinary teams, educate patients and families, and maintain precise documentation, ensuring safe, evidence-based care across acute and rehabilitative settings.

Qualifikationen

• Active Physical Therapist (PT) license in the state of practice.
• BLS/CPR certification preferred or required based on facility policy.
• DPT degree or equivalent educational requirements as required by state regulations.

Aufgaben

• Evaluate patients and develop individualized treatment plans based on functional needs and physician referrals.
• Provide therapeutic interventions to improve mobility, strength, balance, flexibility, coordination, pain management, and functional independence.
• Monitor patient progress, modify treatment plans as appropriate, and document all evaluations, treatments, and outcomes accurately.
• Educate patients, families, and caregivers regarding treatment plans, home exercise programs, injury prevention, and discharge recommendations.
• Collaborate with physicians and interdisciplinary healthcare teams to coordinate patient care and achieve optimal rehabilitation outcomes.
• Maintain accurate and timely clinical documentation in accordance with facility policies and regulatory requirements.
• Ensure compliance with patient safety, infection prevention, and quality assurance standards.
• Treat patients across assigned care settings and age groups as required by the facility.
• Perform other job-related duties as assigned.
