Job summary

Talented Medical Solutions seeks a compassionate Physical Therapist (PT) to provide high-quality rehabilitation services in diverse healthcare settings. You will evaluate patients, create individualized treatment plans, and implement therapeutic interventions to restore mobility and reduce pain, while monitoring progress and documenting outcomes.

Qualifications

• Active PT license in the state of practice.
• Current BLS/CPR certification preferred or required by facility policy.
• Doctor of Physical Therapy degree or equivalent as required by state regulations.
• Experience in acute care, outpatient, inpatient rehab, SNF, home health, or specialty settings preferred.
• Strong clinical assessment, communication, documentation, and patient education skills.

Responsibilities

• Evaluate patients and develop individualized treatment plans based on functional needs and referrals.
• Provide therapeutic interventions to improve mobility, strength, balance, and independence.
• Monitor progress, modify plans as needed, and document evaluations and outcomes.
• Educate patients, families, and caregivers on plans, home programs, and discharge recommendations.
• Collaborate with physicians and care teams to coordinate patient care.
• Maintain accurate clinical documentation in line with policies and regulations.
• Ensure compliance with patient safety, infection prevention, and quality standards.
• Treat patients across assigned care settings and age groups as required.
• Perform additional duties as assigned.
