Job summary

Talented Medical Solutions is seeking a compassionate Physical Therapist (PT) to provide high-quality rehabilitation services in a healthcare setting. Qualifications

• Active PT license in the state of practice.
• BLS/CPR certification preferred or required by facility policy.
• DPT degree or equivalent educational requirements as required by state regulations.
• Experience in acute care, outpatient, inpatient rehab, SNF, home health, or specialty settings preferred.
• Strong clinical assessment, communication, documentation, and patient education skills.
• Ability to work independently and collaboratively within an interdisciplinary healthcare team.

Responsibilities

• Evaluate patients and develop individualized treatment plans based on functional needs and physician referrals.
• Provide interventions to improve mobility, strength, balance, flexibility, coordination, pain management, and functional independence.
• Monitor progress, modify plans as appropriate, document evaluations and outcomes.
• Educate patients, families, and caregivers on plans, home exercise programs, discharge recommendations.
• Collaborate with physicians and teams to coordinate patient care and outcomes.
