Job summary

Talented Medical Solutions seeks a compassionate Physical Therapist (PT) to provide patient-centered rehabilitation in a clinical setting. You will evaluate patients, develop individualized treatment plans, implement therapeutic interventions, and monitor progress to restore mobility and function.

You will collaborate with physicians, nurses, and interdisciplinary teams to deliver safe, evidence-based care.

Qualifications

• Active PT license in Maryland.
• BLS/CPR certification as required by facility policy.
• DPT degree or equivalent education as required by state regulations.
• Experience in acute care, outpatient, inpatient rehabilitation, SNF, home health, or specialty settings preferred.
• Strong clinical assessment, communication, documentation, and patient education skills.
• Ability to work independently and collaboratively within an interdisciplinary healthcare team.

Responsibilities

• Evaluate patients and develop individualized treatment plans based on functional needs and physician referrals.
• Provide therapeutic interventions to improve mobility, strength, balance, flexibility, coordination, pain management, and functional independence.
• Monitor patient progress, modify treatment plans as appropriate, and document all evaluations, treatments, and outcomes accurately.
• Educate patients, families, and caregivers regarding treatment plans, home exercise programs, injury prevention, and discharge recommendations.
• Collaborate with physicians and interdisciplinary healthcare teams to coordinate patient care and achieve optimal rehabilitation outcomes.
• Maintain accurate and timely clinical documentation in accordance with facility policies and regulatory requirements.
• Ensure compliance with patient safety, infection prevention, and quality assurance standards.
• Treat patients across assigned care settings and age groups as required by the facility.
• Perform other job-related duties as assigned.
