Qualifications

• Active PT license in the state of practice.
• DPT degree or equivalent as required by state regulations.
• Experience in acute care, outpatient, inpatient rehabilitation, SNF, home health, or specialty settings preferred.
• Strong clinical assessment, communication, documentation, and patient education skills.
• Ability to work independently and collaboratively within an interdisciplinary healthcare team.

Responsibilities

• Evaluate patients and develop individualized treatment plans based on functional needs and physician referrals.
• Provide interventions to improve mobility, strength, balance, flexibility, coordination, pain management, and functional independence.
• Monitor progress, modify plans as needed, and document evaluations, treatments and outcomes.
• Educate patients, families and caregivers on plans, home programs, injury prevention, and discharge recommendations.
• Collaborate with physicians and teams to coordinate patient care and ensure optimal rehab outcomes.
• Maintain accurate and timely clinical documentation per policy and regulations.
• Ensure patient safety, infection control and quality assurance standards.
• Treat patients across assigned care settings and age groups as required by facility.
• Perform other duties as assigned.
