Job summary

Carle Health is seeking a licensed Physical Therapist to evaluate and treat patients of all ages, modify plans of care as needed, and serve as a clinical resource for staff. Responsibilities include planning and adjusting care, educating patients and families, performing examinations, and documenting care in the EMR.

Qualifications

• Must hold a Bachelor’s degree in Physical Therapy.
• Current license as Physical Therapist (PT).
• BLS certification within 30 days of hire.

Responsibilities

• Demonstrates ability to perform patient transfers with assistance as needed.
• Educates patients, family, and caregivers on plan of care and home programs.
• Evaluates clinical findings to determine physical therapy diagnoses and outcomes.
• Completes daily documentation and charging in therapy EMR.
• Applies therapeutic modalities as appropriate and communicates with referring providers.
