Job summary

The role includes a sign-on bonus and relocation assistance, with responsibilities spanning transfers, education, documentation, and collaboration with referring providers. Licensure in PT and BLS are required, with ongoing CEU for stroke care excellence.

Qualifications

• Licensed Physical Therapist (PT) - Illinois Department of Financial and Professional Regulation (IDFPR)
• Basic Life Support (BLS) within 30 days - American Heart Association (AHA)
• Attend and satisfactorily complete all required continuing education regarding the care of acute stroke patients

Responsibilities

• Performs patient transfers with assistance and uses safe body mechanics.
• Treats patients with dignity, empathy, and engagement, staying fully present.
• Designs and adjusts plan of care based on goals, treatment, outcomes and discharge planning.
• Educates patients, families, caregivers, students, and other health care providers.
• Evaluates findings to determine physical therapy diagnoses and outcomes of care.
• Performs examinations and selects appropriate methods relevant to chief complaint and history.
• Documents according to CTSCP67.
• Screens patients to determine therapy needs and monitors progress by staff support.
• Supervises support personnel and provides feedback.
• Completes annual education for DNV Hip and Knee program requirements.
