Job summary

Great Plains Post Acute in Wichita, KS is seeking a Staff Physical Therapist to evaluate residents, develop treatment plans, and collaborate with a multidisciplinary health team to support rehabilitation goals.

You will supervise PT assistants, provide in-service training, and document care in the medical records, while promoting safety and family communication.

Qualifications

• Bachelor's degree in Physical Therapy; masters/doctorate preferred.
• Licensed to practice Physical Therapy in the state.
• CPR certification required by licensure; continuing education as applicable.
• Proficient with Net Health and Point Click Care software.

Responsibilities

• Evaluate residents to determine PT needs and rehabilitation potential.
• Develop and update individualized treatment plans to improve mobility and independence.
• Supervise PT assistants and student trainees in resident care.
• Communicate progress, goals, and discharge plans with physicians, families, and staff.
• Complete timely, accurate documentation and participate in care conferences.
