Job summary

Grandview Post Acute is seeking a Staff Physical Therapist to evaluate residents, develop treatment plans, and collaborate with a multidisciplinary team to improve mobility and independence.

You will supervise PT assistants, communicate progress with physicians and families, and maintain compliant documentation in Net Health and PointClickCare. A state license and CPR may be required, with continuing education encouraged.

Qualifications

• Bachelor’s degree in Physical Therapy and state licensure.
• Must maintain license in good standing; CPR certification if required by licensure.
• Continuing education requirements per state practice.
• Proficiency with Net Health and PointClickCare systems.
• Able to read and interpret policy and procedure manuals.

Responsibilities

• Evaluate residents to determine PT needs and rehabilitation potential.
• Develop and update individualized treatment plans to improve mobility and independence.
• Supervise/PTA and aides in resident care.
• Communicate progress and discharge planning with physicians, nursing staff, residents, and families.
• Complete accurate documentation per facility and payer requirements.
