Job summary

The Staff Physical Therapist evaluates and treats residents, communicates with families, physicians and other health team members, and maintains documentation of services in the medical records. • Bachelor’s degree in Physical Therapy; Master’s or Doctorate preferred.
• Licensed as a Physical Therapist in the state; maintain licensure.
• Proficient with Net Health and Point Click Care systems.

Responsibilities

• Evaluate residents to determine PT needs and rehabilitation potential.
• Develop and update individualized treatment plans to improve mobility and independence.
• Supervise PT Assistants in resident care and related activities.
• Communicate with physicians, nursing staff, residents, and families about progress and discharge planning.
• Assist with staff training and participate in quality improvement initiatives.
