Job summary

Skyline Heights Nursing and Rehabilitation is seeking a Staff Physical Therapist to evaluate residents, develop treatment plans, and supervise PT aides. Must be licensed as a Physical Therapist in the state and have a degree in Physical Therapy. CPR may be required; experience with Net Health and Point Click Care is a plus.

Qualifications

Responsibilities

• Evaluate residents to determine PT needs and rehab potential.
• Develop and update individualized treatment plans.

• Maintain accurate documentation per facility and payer requirements.
• Participate in care conferences and quality initiatives.
