Job summary

LiveWell Homecare Agency seeks a Physical Therapist to assess, plan, and implement rehabilitative programs enhancing mobility, reducing pain, and increasing strength for patients across care continuum.

You will collaborate with physicians, educate patients and families, and document care with strict adherence to protocols. Continuous professional development and licensure compliance are expected to maintain high-quality care.

Qualifications

• Graduate from an accredited physical therapy program (Bachelor’s, Master’s, or Doctorate).
• Current licensure and certifications, including BLS.
• Effective communication, customer service orientation, teamwork, and basic computer skills.

Responsibilities

• Collaborate with physicians to establish and evaluate treatment plans, goals, and discharge strategies.
• Implement and adjust treatment activities, providing patient and family education and discharge instructions.
• Document evaluations, treatment sessions, and discharge plans following departmental protocols.
• Maintain ethical standards, confidentiality, punctuality, and effective communication.
• Participate in staff meetings, training, and process improvement activities.
• Promote positive patient and team interactions, demonstrating excellent customer service and teamwork.
• Engage in ongoing professional development, completing required competencies and licensure requirements.
