Job summary

REHAB STRATEGIES seeks a dedicated Physical Therapist to assess, develop, and implement care plans that improve mobility and function. You will work across inpatient units, outpatient clinics, home health, and community programs, delivering evidence-based physiotherapy and patient education.

You will collaborate with multidisciplinary teams, maintain EMR documentation, participate in discharge planning, and stay current with advances in rehabilitation techniques to enhance patient outcomes.

Qualifications

• Bachelor’s or Master’s degree in Physiotherapy or equivalent.
• Current licensure/registration as Physical Therapist.
• Experience in inpatient, outpatient, home health, or community care settings.

Responsibilities

• Conduct thorough patient evaluations and develop tailored treatment plans.
• Apply physiotherapy techniques (manual therapy, trigger point therapy, exercises, modalities).
• Collaborate with multidisciplinary teams for discharge planning and care transitions.
• Educate patients and families on injury prevention, procedures, and home exercise programs.
• Document progress in EMR and maintain care plans and notes.
• Monitor responses and adjust treatments for optimal outcomes.
