Rehab Without Walls Neuro Rehabilitation is seeking an experienced Physical Therapist to deliver real-world, home- and community-based therapy that helps patients apply skills in everyday life. Qualifications

• Minimum of bachelor’s degree in Physical Therapy from an accredited program.
• Current, unrestricted Physical Therapist license in the practicing state.
• Current Basic Life Support or CPR certification as required by state regulations.
• A minimum of one year’s work experience as a physical therapist preferred.
• Demonstrates knowledge of rehabilitation techniques related to neurological injury.
• Communicates effectively and professionally in verbal and written interactions.
• Ability to lift up to 50 pounds; patient lifting up to 100 pounds with safety procedures.
• Duties require fine motor skills, visual acuity, and standing for extended periods.

Responsibilities

• Perform evaluations and develop treatment plans.
• Regularly re-assess effectiveness of treatment plans, attend staffing meetings and family conferences.
• Perform discharge evaluations, make recommendations for home and community safety, and provide resources.
• Communicate patient’s needs and progress to the treatment team, physician, patient and family members.
• Coordinate with other disciplines to enhance patient outcomes.
