Job summary

Rehab Without Walls Neuro Rehabilitation is seeking a licensed Physical Therapist to provide functional rehabilitation in home and community settings. You will evaluate patients, build treatment plans, and progress care in real-life environments like kitchens, stores, and workplaces.

You will work autonomously within a collaborative therapist team, delivering care across home, school, work, and community contexts.

Qualifications

• Bachelor's degree in Physical Therapy from an accredited program.
• Current, unrestricted physical therapy license in the practicing state.
• Current CPR/BLS certification as required.
• Minimum one year clinical PT experience preferred.
• Knowledge of neurorehabilitation approaches and safety procedures.

Responsibilities

• Perform evaluations and develop treatment plans.
• Regularly reassess effectiveness of treatment plans and participate in staffing meetings.
• Conduct discharge evaluations and provide home/community safety recommendations.
• Communicate patient needs and progress to the treatment team, physicians, patients, and families.
• Coordinate with other disciplines to ensure collaborative care and positive outcomes.
