Job summary

Rehab Without Walls Neuro Rehabilitation is expanding its home- and community-based neuro rehabilitation team. As a Physical Therapist, you will evaluate and treat patients with neurological injuries in real-world settings, tailor plans to individual needs, and collaborate with families and an interdisciplinary team to maximize independence.

The role emphasizes autonomy, direct patient care in home environments, and opportunities for professional growth within a supportive network.

Qualifications

• Bachelor’s degree in Physical Therapy from an accredited program.
• Current, unrestricted Physical Therapist license in the practicing state.
• BLS/CPR certification per state regulations.

Responsibilities

• Perform evaluations and develop treatment plans.
• Regularly reassess effectiveness and participate in staffing meetings with families.
• Perform discharge evaluations and provide home safety recommendations and resources.
• Communicate patient needs and progress to the treatment team and family.
• Collaborate with other disciplines to optimize patient outcomes.
