Job summary

Centra Health is seeking an Activity Program Coordinator to plan and implement the center's social, recreational, and therapeutic activity programs. You will supervise Activity Assistants, coordinate care with the Interdisciplinary Team, and act as liaison between the team, staff, participants, and program members.

Qualifications

• High School Diploma, GED, or Equivalent is required.
• 1 year of experience working with the elderly and/or those with disabilities.

Responsibilities

• Completes all regulatory assessments and plans of care.
• Develops, implements, conducts, and/or supervises the centers activity programs to meet physical, cognitive, social, and psycho-social needs of participants, considering functional limitations.
• Promotes and coordinates the volunteer program for the center.
• Creates and maintains an environment that protects participants from harm and promotes wellbeing.
• Communicates and collaborates with the interdisciplinary team, staff, and participants/ individuals.
• Handles conflict resolution opportunities in an appropriate, timely, and consistent manner.
• Operates the activity program within budget.
