Job summary

Centerville Post Acute is seeking an Activity Assistant to support the Activity Director in planning and running a creative activities program that meets residents' physical, mental, and psychosocial needs. You will help coordinate individual, small and large group activities and assist with calendars and attendance.

You will work to ensure clear communication among staff, residents, families, and agencies, and assist in discharge planning and activity care plans.

Qualifications

High school diploma or equivalent.
Preferable one-year experience in a long-term care facility.
Ability to read technical procedures.
Ability to read and comprehend policy and procedure manuals.
Ability to present information and respond to questions.
Ability to apply fractions, percentages, ratios and proportions.
Ability to interpret written, oral, diagram, or schedule instructions.

Responsibilities

Participate in planning and conducting activities for residents.
Assist in communication between staff, residents, families, support personnel, government agencies, and the public to meet residents' needs.
Assist in developing monthly activity calendar and maintaining attendance records.
Assist in discharge planning and developing activity care plans.
Keep activity area clean, orderly and secure.
