Job summary

Centerville Place Assisted Living is seeking an Activities Assistant to support the planning, coordination, and execution of a wide range of resident activities. The role involves creating calendars, coordinating transportation, and ensuring activities align with each resident's care plan.

You will interview residents or families for preferences, assist with in-room programs, and document participation and responses.

Qualifications

• High school diploma or equivalent.
• Must be able to pass background check.

Responsibilities

• Assist in planning, organizing, implementing, and evaluating recreational, social, intellectual, emotional and spiritual programs.
• Create a monthly calendar of activities and post in a prominent location.
• Ensure residents' calendars are in their rooms.
• Interview residents or family members to obtain activity preferences.
• Coordinate and assist residents in getting to activity locations within the facility.
• Assist in conducting activities to promote resident well-being (physical, social, mental).
• Document attendance and chart progress notes; indicate resident response to services.
• Observe mood/behavior and report changes to the Activities Director.
• Involve residents and families in planning activities when possible.
• Assist with in-room activities and one-on-one visits.
• Coordinate offsite field trips and transportation when necessary.
• Coordinate needs with other departments to avoid interruptions.
• Assist with Resident/Family Council meetings as indicated.
• Other duties as assigned.
