Job summary

Harborview Medical Center in Seattle is seeking a Social Worker Assistant - Inpatient Transportation to coordinate early discharge transports from hospital visits to home, SNF, AFH or LTAC. You will act as liaison between Hopelink, Lyft, taxi vendors and community agencies to support smooth transitions.

The role includes gathering and maintaining data, assisting with discharge planning, and using tools like Provider One and EPIC to keep the care team informed and aligned.

Qualifications

• Bachelor of Social Work or a Bachelor’s degree in the behavioral sciences with two years of social work experience.
• OR An Associate of Arts Degree in the behavioral sciences with four years of social work experience.
• Equivalent education and/or experience may substitute for minimum qualifications where there are no legal requirements such as a license or certification.

Responsibilities

• Serve as an effective liaison between clinics, inpatient units, outside providers, and SW Supervisors.
• Understand hospital workflows including discharges, clinic appointments and ER visits.
• Demonstrate innovative problem-solving in discharge planning.
• Communicate discharge plans to the care team as needed.
• Maintain access and working knowledge of tools such as Provider One, Lyft, and EPIC.
