Job summary

Qualifications

• Two years' experience in the social work field.
• Equivalent education/experience may substitute where required.

Responsibilities

• Serve as liaison between clinics, inpatient units, outside providers, and SW Supervisors.
• Understand hospital workflows (discharges, clinic appointments, ER visits).
• Demonstrate innovative problem solving and communicate discharge plans to the team.
• Maintain access to tools used daily (Provider One, Lyft, EPIC).
