Position Highlights
  • The role provides care coordination for effective early discharge.
  • Two‑year funded State project focusing on Medicaid/Managed Medicaid/Dual eligible adults ready for transfer but facing transition barriers.
  • The Social Worker collaborates with patients and interdisciplinary teams to identify clinical and financial discharge targets and eliminate barriers.
Primary Job Responsibilities
  • Provide specialty consultation to staff and health care teams regarding psychosocial aspects of client care.
  • Consult to Care Management and other staff who refer to the ECM program.
  • Offer psychosocial support to patients and families to facilitate discharge from inpatient to ECM and post‑acute settings.
  • Follow patients in post‑acute settings and coordinate with interdisciplinary team for ongoing clinical needs and community discharge planning.
  • Visit patients at various sites including inpatient, skilled nursing facilities, community dwellings, shelters, etc.
Required Position Qualifications
  • Master of Social Work degree from a Council on Social Work Education accredited program.
  • One year of full‑time social work experience in a health care setting or equivalent (including practicum).
Legal Requirement
  • Within 90 days of hire must hold one of the following Washington state credentials: Agency Affiliated Counselor, Licensed Social Work Associate – Advanced, Licensed Social Work Associate – Independent Clinical, Licensed Advance Social Worker, Licensed Independent Clinical Social Worker.
Compensation & Benefits
  • Pay Range: $43.41 – $67.93 hourly.
