The position requires a Bachelor of Social Work or a Bachelor's Degree in the behavioral sciences with relevant experience, plus data gathering as needed.

Harborview Medical Center – Social Work Department has an outstanding opportunity for a Social Worker Assistant - Inpatient Transportation.

POSITION HIGHLIGHTS
  • Provide transportation coordination to help effect early discharge placements from a hospital/clinic visit to a home, SNF, AFH, LTAC, IPR, etc.
  • The SW Assistant serves as a liaison between Hopelink, Lyft, taxi vendors, and outside community agencies etc. between Skilled Nursing Facilities, Adult Family homes, transportation brokers, outside community agencies etc. as delegated by the Inpatient Social Work Supervisor.
  • As needed, the SW Assistant will assist with gathering and maintaining data as delegated by the department.
Primary Job Responsibilities
  • Serve as an effective liaison between clinics, inpatient units, outside providers, SW Supervisors, etc.
  • Have a good understanding of the different hospital workflows (discharges, clinic appointments, ER visit, etc.)
  • Demonstrate innovative problem solving
  • Communicate discharge plan to appropriate team members as needed
  • Maintain access and working knowledge of different tools used in performing daily functions (Provider one, Lyft, EPIC, etc.)
  • Address discharge plan with the care team as needed
Required Position Qualifications
  • Bachelor of Social Work OR a Bachelor’s Degree in the behavioral sciences AND two years experience in the social work field.

OR

  • An Associate of Arts Degree in the behavioral sciences AND four years of work experience in the social work field.

Equivalent education and/or experience may substitute for minimum qualifications except when there are legal requirements, such as a license, certification, and/or registration.
