Job summary

UW Medical Center-Northwest is seeking a Social Work Assistant on a per diem, part-time on-site schedule in Seattle. You will partner with Social Workers and Nurse Care Coordinators to support discharge planning and coordinate patient care with families and the health care team.

Responsibilities include arranging long-term care placements, arranging home care services, facilitating safe transitions to home or other facilities, and helping families with emergency resources.

Qualifications

• Bachelor of Social Work or a Bachelor’s Degree in the behavioral sciences with two years’ social work experience.
• Associate of Arts Degree in the behavioral sciences with four years’ social work experience.
• Equivalent education/experience may substitute for minimum qualifications, barring licenses or registrations.

Responsibilities

• Facilitate placement in long-term care facilities based on clinical data.
• Arrange home care services such as home health or hospice referrals.
• Assist with safe transitions to home or other facilities.
• Assist families with emergency resources (funds, food vouchers, transportation).
• Respond to referrals from SW/NCCs promptly and document in EMR.
• Prepare letters/documents to aid families in obtaining benefits; copy/fax/mail as needed.
• Provide notary public services to patients and staff.
