Job summary

Penn Medicine is seeking a Registered Nurse Case Manager to coordinate discharge planning, utilization management, and post-acute transitions for patients. Qualifications

• Registered Nurse - PA (Required)
• PA RN Licensure required
• Bachelor of Arts or Science (Required)
• Education Specialization: Nursing; Experience: 2+ years Clinical Experience

Responsibilities

• Utilization management activities: monitor appropriate use of internal resources and coordinate with care team
• Discharge planning: timely coordination of home care, DME, placements, hospice, transportation
• Quality data collection and risk management referral
• Educates patient and family regarding scope and limits of health care benefits
• Communicates discharge time frame to patient and family
• Keeps current with new providers, technologies and treatment modalities
• Assists patient in moving to the next level of care with transfers to home, agencies, and facilities
• Participates in daily interdisciplinary meetings to drive accountability and plan care
