Job summary

Qualifications

• Registered Nurse with PA licensure required.
• Bachelor’s degree (BA/BS) required.
• At least 2 years of clinical experience.

Responsibilities

• Coordinate home care, DME, placements, hospice, community resources, transportation.
• Discharge planning and timely coordination with patient and family.
• Educate patients and families about scope and limits of healthcare benefits.
• Document discharge plan and reflect in EPIC.
• Participate in daily interdisciplinary meetings.
• Keep systems updated (EPIC, Penn Chart, Haiku, Outlook).
